***Hiring part time for evening and weekend shifts***

***Must be able to travel to Westfield Job Sites***

I. Provide hands on personal care including all ADL’s (bathing, oral care, peri care, dressing, feeding, transfer, ambulation, medication administration)

II. Complete cleaning tasks in homes including but not limited to dusting, vacuuming, mopping, sweeping, cleaning bathrooms, kitchens

III. Do laundry as needed

IV. Meal preparation as well as cooking and serving meals depending on the individualized care plan of the client

V. Medication administration as instructed by administration or family at the direction of physician

VI. Document shift thoroughly in Connecteam App following shift and send shift report in group chat for client for each and every shift

VII. Attend required trainings/in-services as necessary

VIII. Clearly communicate with team members

IX. Communicate all client needs/concerns to administrative staff immediately using the 24/7 call line

X. Provide scheduler with accurate availability and work shifts as scheduled
