Position: Home Care Coordinator
Type: Full-Time
Schedule: Monday - Thursday 8:00 AM - 4:00 PM Friday, 8:00 AM - 1:00 PM
Location: Woodcliff lakes, NJ

We are seeking a compassionate and detail-oriented Home Care Coordinator to join our team and help us deliver high-quality care.
Key Responsibilities:
- Oversee and coordinate home care services for clients, ensuring their needs are met with the highest standard of care.
- Develop and maintain care plans in collaboration with clients, families, and healthcare providers.
- Schedule and manage caregiver assignments, ensuring appropriate match between caregivers and clients.
- Monitor client progress and adjust care plans as needed.
- Address client and caregiver concerns and resolve issues in a timely and professional manner.
- Maintain accurate and up-to-date records of care services provided.
- Ensure compliance with all relevant regulations and company policies.
- Work efficiently in a fast-paced environment, managing multiple tasks and priorities effectively.
Qualifications:
- High school diploma or equivalent required; Associate’s or Bachelor’s degree in healthcare, social work, or a related field a plus.
- Previous experience in home care coordination or a related field is highly desirable.
- Strong organizational skills with a keen attention to detail.
- Ability to work quickly and effectively under pressure.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Proficiency in Microsoft Office and other relevant software.
- Must be able to work Mon-Fri 8am-4pm in Woodcliff Lakes office.
