Skilled Nursing & Rehabilitation Facilities

Burlington, IA

Fulltime M-F


Position Summary

We are seeking a compassionate and experienced Nurse Practitioner (NP) or Physician Assistant (PA) to provide high-quality medical care to patients in Skilled Nursing and Rehabilitation Facilities. The Advanced Practice Provider (APP) will evaluate, diagnose, treat, and manage acute and chronic medical conditions while collaborating with facility staff and interdisciplinary teams to improve patient outcomes and reduce hospital readmissions.


Essential Responsibilities

  • Conduct new patient, follow-up, and medically necessary evaluations.
  • Diagnose and manage acute and chronic illnesses.
  • Develop and implement individualized treatment plans.
  • Order and interpret laboratory and diagnostic tests.
  • Prescribe medications and treatments in accordance with state regulations.
  • Collaborate with physicians, nursing staff, therapists, and other healthcare professionals.
  • Complete timely, accurate documentation in the electronic medical record (EMR).
  • Participate in care planning and quality improvement initiatives.
  • Educate patients and families regarding treatment plans and disease management.
  • Ensure compliance with applicable regulatory and clinical standards.


Qualifications

Required

  • Current NP or PA license in the applicable state.
  • National certification (AANP, ANCC, or NCCPA).
  • DEA registration (or ability to obtain).
  • Current BLS certification.
  • Excellent clinical assessment and communication skills.
  • Valid driver's license and reliable transportation.


Preferred

  • Experience in Skilled Nursing, Rehabilitation, Long-Term Care, Geriatrics, Internal Medicine, or Family Practice.
  • Experience with EMR documentation.


Schedule

  • Full-Time 40 hours/week
  • Monday–Friday (1 work from home day)
  • Flexible scheduling
  • Travel between assigned facilities required
