Join a skilled nursing team in Santa Rosa, CA as a Licensed Practical Nurse / Licensed Vocational Nurse (LPN/LVN). This opportunity is ideal for a dependable clinician who is comfortable providing resident-centered care, supporting nursing interventions, and helping maintain a safe, clean, and compassionate care environment.

Essential Functions:

  • Provide quality customer service efficiently to residents, families, co-workers, and vendors in a manner that ensures satisfaction.
  • Arrive to work on time, in a proper clean uniform, with good personal hygiene and nametag visible.
  • Coordinate and perform resident assessments and plan of care evaluations when appropriate.
  • Coordinate and perform needed treatments, medications, or other nursing interventions as indicated by the patient plan of care or as ordered by the physician.
  • Ensure all needed emergency measures are followed in accordance with facility policy and standards of nursing practice.
  • Maintain inventory of medical supplies and medications to meet patient/resident needs.
  • Ensure that all patient/resident rights are protected.
  • Maintain all documentation as required by federal and state regulations and company policy.
  • Perform all duties in a safe and efficient manner.
  • Use equipment correctly and safely when performing patient/resident care.
  • Follow department safety protocols at all times.
  • Wear safety equipment as required for the job.
  • Report any safety hazards and/or accidents to a supervisor.
  • Follow the highest standards of cleanliness.
  • Follow all federal, state, and corporate policies, health codes, and guidelines in preparation and handling of foodstuffs.
  • Comply with all department and facility policies and procedures.
  • Perform other duties as assigned as necessary.

Job Requirements and Qualifications:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill and/or abilities required.   • Current LPN/LVN licensure on file. Experience in long-term care community health, geriatric and/or rehabilitative nursing desirable. Knowledgeable in current State and Federal regulation.
  • Must be able to read, write, speak and understand English.
  • Basic mathematical skills required.
  • Must be able to understand, follow, support and initiate policies and procedures of the facility.
  • Must be able to recognize emergency situations and respond appropriately.
