Job Summary:
The Registered Nurse (RN) is responsible for delivering comprehensive nursing care to patients in a post-acute care setting. This role includes assessing patient health, developing and implementing nursing care plans, and collaborating with interdisciplinary teams to provide high-quality patient care.
Key Responsibilities:
- Conduct detailed assessments of patient health conditions and monitor ongoing health status.
- Implement individualized care plans in coordination with healthcare providers.
- Administer medications and treatments as prescribed, monitoring for side effects or adverse reactions.
- Provide patient and family education on disease processes, treatment protocols, and self-care.
- Ensure accurate and timely documentation of patient information, including vital signs, progress notes, and medical history.
- Collaborate with physicians, therapists, social workers, and other team members to achieve the best patient outcomes.
- Participate in discharge planning, ensuring a smooth transition to home care or other appropriate settings.
- Adhere to all safety, infection control, and quality standards set by regulatory agencies and the organization.
Qualifications:
- Active RN license.
- Minimum of 1-2 years of nursing experience, preferably in post-acute, rehabilitation, or long-term care.
- CPR/BLS certification.
