\n\nWe are hiring an Admission Registered Nurse (RN) for a Hospice position providing end-of-life care to patients in their homes in the Mechanicsburg, PA area. This position is eligible for a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year!
\n\nWhat You’ll Do:
\n- \n
- Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life \n
- Conduct comprehensive hospice admission assessments, including clinical evaluations, medication reconciliation, and development of individualized plans of care aligned with physician orders \n
- Educate and guide patients and families through the hospice admission process, setting expectations, explaining the hospice philosophy, and ensuring they feel informed, supported, and comfortable with the transition to end-of-life care \n
- Apply strong clinical judgment and compassionate critical thinking to facilitate timely admissions, address immediate needs, and ensure accurate, thorough, and compliant EMR documentation \n
- Initiate evaluations to other disciplines when indicated \n
- Work independently, but never alone \n
- Travel to patient homes within assigned territory \n

Admission RN Qualifications
\n- \n
- Active Registered Nurse, RN license in the state you work \n
- Two years of RN experience and the clinical expertise to deliver quality patient care, hospice experience preferred \n
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations \n
