Home Health PT - Medford OR and Surrounding Areas

Job Details:

\n

• 8-hour day shifts (4x10 and 3x12 schedules cannot be accommodated)
\n• May visit up to 6 patients per day with 30 productivity points as standard expectation
\n• 1:1 patient ratio
\n• No weekend requirements
\n• No on-call requirements
\n• Facility may call off up to one shift per pay period (every two weeks)
\n• Minimum 1 year of skilled home health experience required
\n• First-time travelers not accepted
\n• Oasis experience required
\n• Required certification: BLS
\n• Traveler must have reliable personal transportation
\n• Occasional maximum travel radius of 50+ miles, though system groups clients to minimize drive and windshield time
\n• Charting system: EPIC/Oasis
\n• Any color scrubs acceptable
\n• Free parking provided
\n• Mileage reimbursement at IRS standard rate
\n• Relevant skills include acute care, outpatient clinic, rehabilitation, skilled nursing, orthopedic conditions, neurological conditions, post-stroke rehabilitation, spinal cord injury, traumatic brain injury, home exercise programs, and patient/family education
