Job Details:

• Provide physical therapy services across assigned county territories including Area #1 (Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City) and Area #2 (Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont)
\n• Additional travel outside of assigned areas may be required
\n• Weekend coverage may be required
\n• Guarantee option requires a minimum of 27 points per week with a 36-hour guarantee
\n• Traveler must be willing to travel outside the service area or work weekends to maintain weekly visit expectations under the guarantee option
\n• Guarantee does not apply for the first and last week of assignment, weeks with holidays, or weeks with approved time off
\n• Non-guarantee option allows patient refusal with no guaranteed hours and payment made on a per visit basis
\n• Visit types include OASIS ReCertification, OASIS RoC, Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/up, Re-Eval, Eval Discharge, OASIS Discharge, and OASIS F/up
\n• Rate is all-inclusive and covers drive/travel time, visit time, documentation time, and time spent making phone calls to patients and doctors
\n• Required credentials: Current California PT License, BLS, Driver's License, Auto Insurance, Auto Registration
