American Traveler is seeking an experienced RN - Case Manager for a hospital-based position in California, requiring an active CA RN license and a minimum of 2 years of case management experience.
Details- Hospital acute care setting, Case Management unit
- Day shift schedule, Monday through Friday, 8:00 AM – 4:30 PM (5x8-hour shifts)
- Focuses on utilization management, resource management, discharge planning, and post-acute care referrals across the care continuum
- Manages complex acute patient populations with a goal of preventing unnecessary admissions and readmissions
- Provides discharge planning coordination and intervention for high-risk patients presenting to the Emergency Department
- 13-week contract
- Active California (CA) RN license required
- Current BLS certification required
- Minimum 2 years of RN experience required
- Coordinates integrated and coordinated care to promote patient wellness, improved outcomes, efficient utilization of health services, and minimized payment denials
- Candidates living within 50 miles of the facility must contact their recruiter regarding local rate eligibility
- Former employees or travelers must have been separated from the facility for at least 1 year before returning as a traveler
- Only 1 holiday RTO request will be approved during the contract
- 2 supervisor references obtained within the past year are required for consideration
- Copies of all certifications are required for consideration
