We are seeking a dedicated and compassionate LPN/RN to join our team at St. Luke's Hospital Transitional Care Center in Cedar Rapids, IA. This position is for 1st shift, providing care and support to our residents in a part-time capacity.

Job Responsibilities:

• Provide direct patient care, including administering medications, monitoring vital signs, and assisting with activities of daily living
• Collaborate with the interdisciplinary team to develop and implement care plans
• Communicate effectively with patients, families, and other healthcare team members
• Maintain accurate and up-to-date patient records
• Ensure a safe and supportive environment for patients

Qualifications:

• Active LPN/RN license in the state of Iowa
• Previous experience in long-term care or transitional care settings preferred
• Strong communication and interpersonal skills
• Ability to work effectively in a team environment
• Compassionate and patient-centered approach to care
