We are hiring a Medical Social Worker (MSW) for a Home Health position providing skilled nursing care to patients in their homes in the Elmhurst and surrounding areas.
What You’ll Do:
- Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
- Conduct comprehensive psychosocial assessments, identify patient and family needs, and develop individualized care plans that address emotional, social, and environmental factors
- Educate and support patients and families by connecting them with community resources, counseling services, and care planning guidance to ensure they feel informed, supported, and empowered
- Apply critical thinking and problem-solving skills to navigate complex situations, advocate for patient needs, and maintain accurate, timely, and compliant EMR documentation
- Work independently, but never alone
- Travel to patient homes within assigned territory

MSW Qualifications
- Master’s in Social Work from graduate school accredited by the Council of Social Work
- One (1) to two (2) years post graduate social work experience in a health care setting
- Must hold, in good standing, a license as a Medical Social Worker issued by the state in which you work
- Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
