Responsibilities
- Administer prescribed medications and treatments per physician orders and document administration in the chart.
- Monitor residents’ conditions, assess for changes, and report findings to the supervising RN.
- Perform routine nursing tasks such as vital signs, wound care, dressing changes, and routine injections within scope of practice.
- Assist residents with activities of daily living and mobility, ensuring safety and comfort.
- Collaborate with licensed staff to implement and adjust individualized care plans.
- Maintain accurate, timely documentation and ensure compliance with regulatory standards.

Requirements
- Active Licensed Practical Nurse license in New Hampshire.
- Ability to work in a long-term care setting and participate as part of an interdisciplinary team.
- Availability for a travel temporary assignment with guaranteed 40 hours per week.
- Ability to work flexible shifts, including nights, as scheduled.
- Strong communication, documentation, and patient-care skills.
- Must comply with applicable policies and regulatory requirements.
