About the Role:

The FASNY Firefighter's Home, a premier skilled nursing facility in Hudson, New York is seeking two part-time Licensed Practical Nurses. We are hiring an individual to join teams working on the 3:00 - 11:00pm shift and another to join our 11:00 pm - 7:00 am shift. Individuals working these shifts receive additional shift differentials.

The Licensed Practical Nurse (LPN) plays a critical role in delivering high-quality nursing care for our residents. This position focuses on providing direct patient care, including wound management, pain control, and ensuring patients' comfort and safety. The LPN will collaborate closely with Unit Managers to implement individualized care plans and educate residents and their families on health maintenance and treatment protocols. Effective communication in English, both written and verbal, is essential to document patient progress accurately and to provide clear instructions and education. Ultimately, the LPN contributes to improving resident outcomes by delivering compassionate, skilled nursing care and supporting the overall healthcare team in a dynamic environment.

Minimum Qualifications:

  • Valid and current Nursing License as a Licensed Practical Nurse (LPN) in the United States.
  • Proficiency in reading, writing, and speaking English to effectively communicate with patients and healthcare team members.
  • Demonstrated experience in patient care, including wound care and pain management.
  • Ability to perform basic nursing procedures.
  • Completion of an accredited practical nursing program.

Preferred Qualifications:

  • Experience working in an assisted living facility or similar long-term care environment.
  • Familiarity with electronic health record (EHR) systems.

Responsibilities:

  • Administer medications and pain management interventions as prescribed, monitoring patient responses and reporting any concerns.
  • Provide comprehensive wound care support, including dressing changes, and monitoring for signs of infection.
  • Educate patients and their families on health conditions, treatment plans, and preventive care measures.
  • Collaborate with Nurse Managers and interdisciplinary teams to develop and update individualized patient care plans.
  • Maintain accurate and timely documentation of patient care activities, observations, and communications in compliance with regulatory standards.
  • Support daily resident care activities within assisted living facilities, ensuring a safe and supportive environment.

Skills:

The required skills such as wound care, resident education, and pain management are applied daily to assess patient conditions, administer treatments, and provide guidance to patients and their families to promote healing and comfort. Effective communication skills in English are essential for documenting patient information clearly and coordinating care with the healthcare team. The nursing license validates the candidate’s competency and adherence to professional standards, enabling them to deliver safe and ethical care. Preferred skills, including experience in assisted living facilities and collaboration with Nursing Leadership, enhance the nurse’s ability to work within interdisciplinary teams and adapt to the specific needs of long-term care populations.
