About the Opportunity
A mission-driven Federally Qualified Health Center (FQHC) in Brooklyn, NY is seeking a compassionate and patient-focused Physician to join its Adult Medicine team. This is an excellent opportunity for a physician who is passionate about providing high-quality, community-based healthcare to underserved populations while working within a collaborative, multidisciplinary environment.
We welcome physicians with 1+ years of experience in primary care, family medicine, internal medicine, community health, or FQHC settings. Newer physicians with a strong commitment to community medicine are encouraged to apply.
Position Summary
The Physician will provide comprehensive primary care services to adult patients, including preventive care, chronic disease management, health education, and coordination of specialty services. The physician will work closely with nurse practitioners, behavioral health providers, care coordinators, and support staff to deliver patient-centered care that improves health outcomes and enhances access to quality healthcare.
Responsibilities
- Provide comprehensive primary care services to adult patients.
- Diagnose and treat acute and chronic medical conditions.
- Manage conditions such as diabetes, hypertension, asthma, COPD, and cardiovascular disease.
- Perform routine physical examinations and preventive health screenings.
- Order, interpret, and communicate laboratory and diagnostic test results.
- Develop individualized treatment plans and monitor patient progress.
- Educate patients on disease prevention, wellness, and healthy lifestyle choices.
- Coordinate referrals and collaborate with specialists and community resources.
- Maintain accurate and timely documentation within the electronic medical record (EMR).
- Participate in quality improvement initiatives and population health programs.
- Support the organization's mission of providing accessible, culturally competent healthcare services.
Qualifications
Required
- MD or DO degree from an accredited medical school.
- Board Certified or Board Eligible in Internal Medicine or Family Medicine.
- Active New York State medical license (or ability to obtain).
- DEA registration (or ability to obtain).
- Minimum of 1 year of clinical experience in primary care, outpatient medicine, community health, FQHC, or a similar setting.
- Strong clinical, communication, and interpersonal skills.
Preferred
- Experience working with medically underserved or diverse patient populations.
- Bilingual skills (Spanish, Russian, Mandarin, Haitian Creole, or other languages commonly spoken in Brooklyn) are a plus.
- Experience with value-based care, quality metrics, and population health initiatives.
