Certified Home Nursing Solutions LLC (CHNS) is seeking a compassionate and motivated Employment Services Specialist (DDA) to support individuals with intellectual and developmental disabilities (IDD) in achieving meaningful, competitive employment and greater independence.
What You'll Do
As part of our DDA Employment Services team, you will:
Conduct Discovery assessments to identify each person's strengths, interests, skills, and career goals.
Develop individualized employment plans using Person-Centered Planning.
Assist individuals with career exploration and vocational goal setting.
Provide job readiness training, including:
Resume development
Employment applications
Interview preparation
Workplace communication
Professional behavior
Develop partnerships with local businesses and employers.
Match individuals with competitive, integrated employment opportunities.
Provide one-on-one job coaching before, during, and after employment.
Teach workplace skills and promote independence on the job.
Support employers with disability awareness and workplace accommodations.
Monitor participant progress and provide ongoing employment retention support.
Complete documentation that complies with Maryland DDA regulations and agency standards.
Participate in Person-Centered Planning meetings and interdisciplinary team meetings.
Collaborate with families, support coordinators, vocational rehabilitation counselors, employers, and community partners.
Travel throughout the community to meet participants at homes, workplaces, and community locations.
Maintain confidentiality and follow HIPAA and agency policies.
Qualifications
High school diploma or GED required; Associate's or Bachelor's degree in Human Services, Social Work, Psychology, Rehabilitation, or a related field preferred.
Experience working with individuals with developmental disabilities preferred.
Knowledge of DDA Employment Services, Supported Employment, and Person-Centered Planning is a plus.
Strong communication, organizational, and documentation skills.
Valid driver's license, reliable transportation, and ability to travel within the community.
Ability to pass required background checks.
Physical Requirements
Ability to sit, stand, walk, bend, and reach for extended periods.
Ability to lift and carry up to 25 pounds.
Ability to travel regularly throughout the community to participant homes, workplaces, and community locations.
Ability to support individuals in various employment and community settings.
