What You'll Do: Provide skilled nursing care to home health patients in accordance with established care plansPerform comprehensive patient assessments and ongoing evaluationsDevelop, implement, and update individualized plans of careMonitor patient conditions and communicate changes to physicians and care team membersCoordinate and oversee patient services to ensure continuity and quality of careEducate patients and families regarding disease management, medications, treatments, and home safetySupervise and direct licensed and non-licensed clinical staff as appropriateComplete timely and accurate clinical documentation and chartingConduct chart reviews for active and discharged patient records to ensure compliance and quality standardsParticipate in after-hours and on-call coverage as assignedCoordinate scheduling and staffing needs as requiredMaintain patient confidentiality and comply with all HIPAA regulationsStay current on nursing best practices, regulations, and clinical innovations #zr
