Job Summary:

We are seeking a compassionate and experienced MDS LVN or RN to join our team at Avir at Mesquite. As a valued member of our interdisciplinary care team, you will play a critical role in providing exceptional care to our patients, promoting their independence, and enhancing their overall quality of life. Responsibilities:

• Conduct comprehensive assessments of patients to identify their unique needs and develop individualized care plans

• Develop, implement, and evaluate care plans in collaboration with the interdisciplinary care team

• Provide direct patient care, including but not limited to: administering medications, performing treatments, and managing medical conditions

• Collaborate with physicians, therapists, and other healthcare professionals to ensure continuity of care

• Educate patients and their families on disease management, medication usage, and other vital health information

• Document patient information accurately and timely in accordance with company policies and procedures

• Participate in quality improvement initiatives to continually improve patient care and outcomes

• Maintain HIPAA compliance and adhere to company policies and procedures

Requirements:

• Current licensure as an LVN or RN in the state of Texas

• Bachelor's degree in Nursing (BACHELORS)

• 0 years of minimum experience (MINIMUM YEARS OF EXPERIENCE)

• Ability to work full-time schedule (FULL_TIME)
