Position Summary Moorings Park is seeking a dedicated and talented Licensed Practical Nurse (LPN) to join our team. The Licensed Practical Nurse is responsible for collaborating with physicians and an interdisciplinary team to provide nursing services to residents. This position also provides support to family members as well as supervising assigned team members. This role is responsible for demonstrating a commitment to customer service through resident focused initiatives that support the mission of the community and Moorings Park.
Key Contributions
- Participates with members of the interdisciplinary team to review, plan, coordinate and evaluate resident’s care. Documents on the resident’s condition and nursing needs accurately and in a timely manner. Reports pertinent observations and reactions regarding residents in a timely manner.
- Develops, directs, and monitors nursing assistant assignments adjusting based on census and level of care required. Communicates staffing problems and needs to supervisor. Escalates team member performance issues to supervisor, participate in counseling and disciplinary action to the extent permitted by the State Nurse Practice Act.
- Executes treatments as necessary while documenting status and observing reactions to medications and treatments.
- Generates, prepares, administrates, and chart all medications.
- Initiates physician orders, verifying all orders received are transcribed accurately in electronic records and treatment plan, administer medications, and provide treatments according to orders.
- Facilitates communication with families regarding change in medications and/or changes in the resident.
- Establishes protocol to ensure care plans are continually reviewed for updated and accurate data that represents the individuality of the resident.
- Coordinates admissions, discharges, and transfers in order to deliver quality customer service.
- Plans resident admission as ordered by physician, perform assessment, verify orders with physician, notify pharmacy, laboratory, and other departments as necessary.
- Introduces an accurate and thorough report to the on-coming shift in order to ensure the continuity of maximum resident care.
- Directs team members in emergency situations and contact appropriate parties as needed.
- Orchestrates investigation, corrects, and follows up on resident and family concerns as needed in a timely manner.
- Complies with applicable federal, state, and local laws, rules, and regulations; maintains knowledge of and follows all company policies and procedures.
Qualifications & Requirements
- Excellent oral and written communication skills in the English language required.
- During emergencies or natural disasters, may be required to remain on duty for the duration of the event.
- Graduate of an accredited Practical or Vocational Nursing (LPN/LVN) program or higher nursing degree required
- Active Florida Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license, or higher nursing licensure, required
- Active Florida CPR Certification is required
- One year’s experience working in a long-term care facility.
