PT - Home Health

\n\n

Job Details:

\n

• Work Monday through Friday, 8:30 AM to 5:00 PM with no weekend or holiday requirements
\n• See 4-6 patients per day with the ability to start shifts from home without stopping at an office first
\n• Travel within a 50-mile radius east and west, approximately 60 miles north up to Sun Valley, and approximately 40 miles south to the ID/NV border (no mileage reimbursement provided)
\n• Treat a multidisciplinary adult patient population consisting of approximately 78% Medicare patients including elderly, rehabilitation, CVA, and inpatient surgical referrals covering orthopedic, surgical, and wound cases (no lymphedema)
\n• Perform admissions, treat existing patients, and complete recertifications including a limited number of assisted living patients
\n• Handle limited oxygen requirement cases
\n• Participate in multidisciplinary meetings every Monday and Wednesday to discuss current admissions
\n• Document using Epic with remote client offshoot
\n• Complete department-specific orientation including Epic classes and up to one week shadowing with another therapist
\n• Supervisors available for support throughout assignment
\n• Must have a minimum of 1 year post-graduate physical therapy experience
\n• Must have home health experience, with post-graduate home health experience preferred
\n• Must have orthopedic experience
\n• First-time travelers welcome
\n• Business casual attire or any color scrubs required
\n• Required certifications: BLS (AHA)
\n• Cancellation policy: 4 shifts per 13-week assignment
