About the Licensed Practical Nurse LPN Role:

We are seeking a compassionate and skilled Licensed Practical Nurse LPN to work in our skilled nursing facility. As an LPN at Pine Haven Nursing & Rehabilitation Center, you will be responsible for:

• Providing direct patient care to our residents, including administering medications, performing vital signs, and monitoring their health status

• Collaborating with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of our residents

• Maintaining accurate and up-to-date medical records and reporting any changes in a resident's condition to the healthcare team

Licensed Practical Nurse LPN Responsibilities:

• Provide high-quality patient care in accordance with established nursing standards and protocols

• Conduct initial and ongoing assessments of residents' physical and emotional needs

• Develop and implement individualized care plans with input from the healthcare team

• Administer medications and treatments as prescribed by physicians

• Maintain a clean and safe environment for our residents

• Participate in quality improvement initiatives to enhance the overall quality of care

Licensed Practical Nurse LPN Requirements:

• Current licensure as a Licensed Practical Nurse (LPN) in the state of New York

• Previous experience in a skilled nursing facility or long-term care setting preferred
