LPN

Work to achieve and maintain continuity of resident care by assisting in planning and exchanging information with other health professionals, as well as family members.

Provide physical and emotional support to the residents.

Observe and report significant changes in resident condition.

Assist with resident admissions, transfers, and discharges.

Assist Physician and RN in performing specialized procedures.

Document changes in condition, physician visits, and provide a monthly summary of resident activities.

Respond to emergency situations competently and calmly, exercise appropriate judgment.

Provide direct supervision to the nurse aides and assist in planning and coordinating quality care.

Complete Incident/Accident Reports as necessary.

Provide general individual resident care and treatments.

Sign on bonus-Full-Time $7500.00, Part-Time $3750.00
