As a Licensed Practical Nurse MDS Specialist at Oasis at the Conch Republic, you will play a vital role in our Assisted Living Facilities, providing exceptional care and support to our residents. We are seeking a highly skilled and compassionate professional to join our team.

Responsibilities:

• Conduct and document Resident Assessments, including MDS assessments, to identify individualized care needs and develop personalized care plans

• Collaborate with the interdisciplinary team to develop and implement care plans that promote resident independence, dignity, and well-being

• Provide direct care to residents, including administering medications, performing vital signs, and providing assistance with activities of daily living (ADLs)

• Monitor and report changes in resident conditions, including signs of decline, and work with the team to implement interventions and improve outcomes

• Maintain accurate and comprehensive records, including clinical notes, treatment plans, and resident data

• Participate in quality improvement initiatives to improve resident care and outcomes

Requirements:

• Current LPN/LVN license in the state of Florida

• 1+ year of experience in long-term care, assisted living, or related healthcare setting

• Strong assessment and clinical skills, with ability to prioritize and manage multiple residents' needs

• Excellent communication and interpersonal skills, with ability to work effectively with residents, families, and healthcare team members

• Ability to work varying shifts, including days, evenings, and weekends

• Compassionate and respectful attitude towards residents and colleagues
