What You'll Do

• Evaluate, diagnose, and treat speech, language, cognitive, communication, and swallowing disorders

• Develop individualized treatment plans that help residents achieve their highest level of independence

• Deliver evidence-based therapy interventions in a skilled nursing and rehabilitation setting

• Accurately document evaluations, treatment plans, progress notes, and discharge summaries

• Collaborate with nursing, therapy, dietary, physicians, and other interdisciplinary team members to achieve the best possible resident outcomes

• Educate residents, families, and caregivers on therapy goals, progress, and home strategies when appropriate

• Promote resident safety, independence, dignity, and quality of life through compassionate, person-centered care

• Maintain compliance with all state, federal, and facility policies, procedures, and documentation standards

What We're Looking For

• Active Colorado Speech Language Pathologist (SLP) license or eligibility for licensure

• Graduate of an accredited Speech Language Pathology program

• Strong communication, assessment, and clinical decision-making skills

• A passion for serving older adults in long-term care and rehabilitation settings
