Evening Shift LPN

Job Summary:

We are seeking a compassionate and skilled LPN to join our evening shift team at Milford Meadows Care Center, a loving and caring community that values kindness, respect, and teamwork. As an Evening Shift LPN, you will play a vital role in providing high-quality resident care to our residents while making a real difference in their lives.

Responsibilities:

• Provide direct care to residents, including administering medications, wound care, and medical treatments as needed

• Conduct vital signs, monitor residents' conditions, and report any changes or concerns to the physicians

• Assist with daily living activities such as bathing, dressing, and grooming, as well as feeding and toileting

• Develop and implement individualized care plans in collaboration with the interdisciplinary team

• Maintain accurate and detailed records of patient care and treatment plans

• Collaborate with other healthcare professionals to ensure seamless delivery of care

• Prioritize and manage competing demands and responsibilities in a fast-paced environment
