• Administer medications and treatments within your scope of practice

• Collaborate with interdisciplinary teams to ensure coordinated resident care

• Monitor resident conditions and report changes to the Director of Nursing

• Promote restorative care and resident independence

• Maintain accurate and timely nursing documentation

• Support and mentor nursing staff to create a positive, resident-centered environment

• Ensure resident safety, comfort, dignity, and well-being

• Current Illinois Licensed Practical Nurse (LPN) license

• Graduate of an accredited nursing program

• Passionate about providing personalized, compassionate care

• Dependable, team-oriented, and committed to quality care
