Per Diem Speech-Language Pathologist

As a Per Diem Speech-Language Pathologist, you will play a vital role in our multidisciplinary team, utilizing your skills and expertise to help our residents achieve their communication and swallowing goals.

About the Role:

As a Per Diem Speech-Language Pathologist, you will be responsible for providing high-quality speech-language pathology services to our residents in a skilled nursing facility setting. You will work closely with our interdisciplinary team to develop and implement individualized treatment plans, conduct comprehensive assessments, and provide evidence-based therapy interventions. Your expertise will be essential in helping our patients overcome communication and swallowing disorders, promoting independence, and enhancing their overall quality of life.

Responsibilities:

  • Conduct comprehensive assessments of residents' communication and swallowing abilities

  • Develop and implement individualized treatment plans in collaboration with the interdisciplinary team

  • Provide evidence-based speech-language therapy interventions, including individual and group therapy sessions

  • Monitor and document resident progress, making adjustments to treatment plans as needed

  • Communicate effectively with residents, families, and healthcare staff to ensure excellent care and support

  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination

Requirements:

  • Master's degree in Speech-Language Pathology (CCC-SLP) from an accredited program

  • Current state licensure and certification as a Speech-Language Pathologist (CCC-SLP)

  • Experience working with older adults and individuals with communication and swallowing disorders

  • Ability to work independently and as part of a team

  • Excellent communication and documentation skills
