About the LPN for Personal Care Role
As a Licensed Practical Nurse (LPN), you will be responsible for:
Providing high-quality nursing care to residents in our Personal Care Home,
Developing and implementing individualized care plans in collaboration with the interdisciplinary care team
Monitoring and reporting changes in resident condition, including any concerns or issues that may arise
Educating residents, families, and caregivers on care instructions
Maintaining accurate and complete records of resident care, including documentation of resident interactions
LPN for Personal Care Requirements
Must be a Licensed Practical Nurse (LPN) with a current license to practice in the state of Pennsylvania
Prior experience in a healthcare setting
Ability to work effectively in a fast-paced environment, prioritizing tasks and managing time efficiently
Excellent communication and interpersonal skills, with the ability to work collaboratively with the interdisciplinary care team
Compassionate and respectful attitude towards residents, families, and colleagues
