About the Role:

As an LPN on our evening shift, you will be responsible for providing high-quality patient care, administering medications, and participating in resident assessments. You will work closely with our experienced team of healthcare professionals to ensure the delivery of comprehensive care that meets the unique needs of each resident. Your attention to detail, excellent communication skills, and commitment to delivering exceptional care will be essential in helping us achieve our mission.

Responsibilities:

• Provide direct patient care, including administering medications, performing vital signs, and assisting with activities of daily living (ADLs)

• Participate in resident assessments, develop care plans, and implement interventions to promote resident independence and well-being

• Collaborate with the interdisciplinary team to ensure comprehensive care that meets the unique needs of each resident

• Maintain accurate and detailed records of resident care, including progress notes and medication administration records

• Contribute to a positive and supportive team environment by maintaining open communication, respecting colleagues, and actively participating in team-building activities

• Demonstrate a commitment to ongoing learning and professional development, staying current with industry trends, best practices, and regulatory requirements
