Always Best Care of Greater Cleveland, a leading provider of senior care services, is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team in Lorain County As a Licensed Practical Nurse (LPN) with our organization, you will have the opportunity to make a meaningful difference in the lives of our clients while working in a dynamic and rewarding environment.

Responsibilities

• Provide high-quality, patient-centered care to clients in their homes, as part of our skilled home health care services

• Conduct patient assessments, and implement interventions as needed

• Collaborate with other healthcare professionals to ensure comprehensive and coordinated care

• Perform wound care, and provide other skilled nursing interventions as ordered by physicians

• Document patient care and progress accurately and thoroughly

• Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions
