Licensed Practical Nurse (LPN) Afternoons

LPN 3-11

Job Summary:

At Liberty Center for Rehabilitation and Nursing, we pride ourselves on "Outstanding medical and rehabilitation services". We are seeking a dedicated and compassionate LPN to join our team, working with us to provide exceptional care to our patients. As an LPN 3-11, you will play a vital role in our nursing staff, working a variety of shifts, including days, evenings, and weekends.

Responsibilities:

• Provide high-quality patient care under the supervision of the licensed nurse, including administering medications, performing treatments, and maintaining accurate records

• Develop and implement individualized patient care plans, in collaboration with the interdisciplinary team

• Participate in care conferences and contributes to the development of patient goals and objectives

• Maintain a safe and clean environment for patients, visitors, and staff

• Communicate effectively with patients, families, and caregivers to ensure seamless care coordination

• Perform routine checks on patients, including vital signs, and report any changes or concerns to the licensed nurse

Requirements:

• Graduate of an approved LPN program with a valid license to practice as an LPN in the state of Pennsylvania

• Minimum of 0 years of experience as an LPN, with a willingness to learn and grow with our team

• Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment

• Ability to lift, transfer, and reposition patients, as needed

Working Conditions:

• Work a variety of shifts, including days, evenings, and weekends

• Work in a fast-paced, dynamic environment, with frequent interruptions and changes in priorities

• Participate in on-call rotation as needed
