LPN Memory Care Director

Job Summary:

We are seeking an experienced and compassionate LPN Memory Care Director to join our dedicated team at Ambiance at Mailand, a premier assisted living and memory care community in Central Florida. As a key member of our leadership team, you will play a vital role in ensuring the highest level of care and support for our residents with memory impairments. Responsibilities:

  • Provide direct and indirect patient care to residents with memory impairments, including wound care, medication administration, and vital sign monitoring

  • Develop and implement individualized care plans in collaboration with the interdisciplinary care team

  • Conduct routine assessments and updates to care plans, identifying and addressing any changes or concerns

  • Lead and supervise a team of certified nursing assistants (CNAs) and caregivers in delivering high-quality care

  • Participate in medication administration, including oversight of medication distribution and monitoring of medication side effects

  • Collaborate with the medical director and other healthcare professionals to ensure safe and effective care

  • Maintain accurate and detailed records of resident care and services

  • Develop and implement policies and procedures for memory care services

  • Identify and address any quality of care issues or concerns, escalating as necessary

Requirements:

  • Current LPN license in the state of Florida

  • High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred

  • Minimum 2 years of experience in long-term care or memory care settings, with at least 1 year in a leadership or supervisory role

  • Strong clinical, leadership, and communication skills

  • Ability to work effectively in a team environment and build positive relationships with staff, residents, and family members

  • Familiarity with dementia care best practices and Alzheimer's Association guidelines
