LPN Memory Care Director

Job Summary:
We are seeking an experienced and compassionate LPN Memory Care Director to join our dedicated team at Ambiance at Mailand, a premier assisted living and memory care community in Central Florida. As a key member of our leadership team, you will play a vital role in ensuring the highest level of care and support for our residents with memory impairments. Responsibilities:
Provide direct and indirect patient care to residents with memory impairments, including wound care, medication administration, and vital sign monitoring
Develop and implement individualized care plans in collaboration with the interdisciplinary care team
Conduct routine assessments and updates to care plans, identifying and addressing any changes or concerns
Lead and supervise a team of certified nursing assistants (CNAs) and caregivers in delivering high-quality care
Participate in medication administration, including oversight of medication distribution and monitoring of medication side effects
Collaborate with the medical director and other healthcare professionals to ensure safe and effective care
Maintain accurate and detailed records of resident care and services
Develop and implement policies and procedures for memory care services
Identify and address any quality of care issues or concerns, escalating as necessary
Requirements:
Current LPN license in the state of Florida
High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred
Minimum 2 years of experience in long-term care or memory care settings, with at least 1 year in a leadership or supervisory role
Strong clinical, leadership, and communication skills
Ability to work effectively in a team environment and build positive relationships with staff, residents, and family members
Familiarity with dementia care best practices and Alzheimer's Association guidelines
