**About the Role**

We are seeking a highly motivated and experienced Speech-Language Pathologist (SLP) to join our team in Fort Atkinson, WI. As an SLP at Rock River Nursing & Rehab, you will provide assessment, diagnosis, and treatment planning for patients with communication and swallowing disorders. Your expertise will include designing and implementing individualized treatment plans, conducting speech and language therapy sessions, and collaborating with other healthcare professionals to ensure comprehensive care.

**Responsibilities**

* Conduct comprehensive assessments to identify communication and swallowing disorders
* Develop and implement individualized treatment plans for patients, including speech and language therapy sessions
* Provide therapy sessions to patients with a range of conditions, including stroke, traumatic brain injury, and neurological disorders
* Collaborate with other healthcare professionals, including occupational therapists, physical therapists, and physicians, to ensure comprehensive care
* Participate in interdisciplinary team meetings to discuss patient progress and develop individualized treatment plans
* Maintain accurate records of patient assessments, treatment plans, and progress
* Stay up-to-date with best practices and evidence-based treatment approaches in speech-language pathology
