Job Summary:

We are seeking a dedicated and compassionate LPN Floor Nurse to join our team at Mount Airy Nursing and Rehab, a leading provider of post-acute care in Mount Airy, Maryland. As an LPN Floor Nurse, you will play a vital role in providing high-quality patient care, working closely with our multidisciplinary team to ensure the best outcomes for our patients.

Responsibilities:

• Provide direct patient care, including administering medications, performing vital signs, and monitoring patients' conditions

• Develop and implement patient care plans, in collaboration with the interdisciplinary team, to meet individual patient needs

• Maintain accurate and detailed records of patient care and progress

• Collaborate with other healthcare professionals to ensure seamless patient care transitions

• Participate in quality improvement initiatives and adhere to regulatory standards

• Provide education and support to patients and their families on care plans, medications, and disease management

Requirements:

• Currently licensed as a Licensed Practical Nurse (LPN) in the state of Maryland

• Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and patient needs

• Strong communication and interpersonal skills, with the ability to work collaboratively with other healthcare professionals

• Ability to maintain confidentiality and adhere to HIPAA guidelines

• Basic computer skills and proficiency with electronic medical records
