Job Summary:

Monument Healthcare and Nursing Center, a premier skilled nursing and assisted living facility, is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team in Scottsbluff, NE. As an LPN Floor Nurse, you will play a vital role in providing high-quality patient care, promoting a safe and positive environment, and collaborating with our interdisciplinary team to deliver exceptional outcomes.

Responsibilities:

• Provide direct patient care, including administering medications, wound care, and performing treatments as ordered by physicians

• Conduct initial and ongoing assessments of patients' physical and emotional needs

• Develop and implement individualized care plans in collaboration with the healthcare team

• Monitor patients' conditions and report changes to physicians and other healthcare professionals

• Maintain accurate and timely records of patient care and treatment

• Participate in quality improvement initiatives to promote patient safety and satisfaction

• Maintain a clean and organized work environment, adhering to infection control guidelines

Requirements:

• Current LPN licensure in the state of Nebraska

• High school diploma or equivalent required; associate's or bachelor's degree in nursing preferred

• 0+ years of experience as an LPN, with a focus on gerontological care

• Possess strong communication, leadership, and problem-solving skills

• Ability to work effectively in a fast-paced environment, prioritizing tasks and managing multiple responsibilities

• Compassionate and empathetic approach to patient care, with a commitment to delivering exceptional outcomes
