We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team on night shift at our Rutland, Vermont facility. As a valued member of our care team, you will play a critical role in delivering high-quality, patient-centered care to our residents.

About the Role:

In this role, you will work closely with our interdisciplinary team to provide comprehensive care to our residents, including assessing, planning, implementing, and evaluating their care plans. You will also be responsible for administering medications, wound care, and performing other essential nursing duties. Your strong communication and interpersonal skills will enable you to effectively collaborate with residents, families, and other healthcare professionals to ensure seamless transitions and optimal outcomes.

Key Responsibilities:

• Provide individualized and holistic care to our residents, addressing their physical, emotional, and social needs

• Assess, plan, implement, and evaluate care plans in collaboration with the interdisciplinary team

• Administer medications, wound care, and perform other essential nursing duties as needed

• Develop and maintain positive relationships with residents, families, and other healthcare professionals

• Maintain accurate and detailed records of resident care and progress

• Participate in quality improvement initiatives to continuously improve our care delivery and outcomes
