Job Summary

We are seeking a Registered Nurse or Licensed Practical Nurse to join our growing team at Jethro Health System. For this role, you will provide high-quality nursing care to patient in their home, ensuring their needs are met with compassion and professionalism.

Home Care LPN Responsibilities

• Assess the patient's condition during every visit and document your observations

• Perform different tasks including vital signs and medication review

• Administer medication as prescribed by the patient’s Physicians

• Perform wound care and assess the progress of healing

• Educate patients and their families on proper home health care strategies and procedures

• Coordinate with Physicians and anyone else involved with the patient’s care plan

• Make recommendations for devices/tools that might improve the patient’s quality of life

• Listen to the patient and respond to concerns or requests.

• Complete all clinical documentation accurately and timely

Qualifications

• Active Licensed Practical Nurse (LPN) license in Massachusetts

  • 1–2 years of clinical experience (home care experience preferred)

  • Strong communication and organizational skills

  • Ability to work independently in the field

  • Familiarity with EMR systems

  • Reliable transportation

• Pay: $30.00 - $35:00 per visit
