About the Role
As an LPN Licensed Practical Nurse, you will:
Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring
Work closely with other healthcare professionals to develop and implement individualized care plans
Conduct initial patient assessments and maintain accurate and up-to-date records
Participate in interprofessional team rounds and maintain open communication with patients, families, and colleagues
Uphold the highest standards of patient safety, dignity, and respect

Responsibilities
Provide competent and compassionate care to patients, including administering medications, wound care, and vital sign monitoring
Collaborate with other healthcare professionals to develop and implement individualized care plans

Participate in interprofessional team rounds and maintain open communication with patients, families, and colleagues
Uphold the highest standards of patient safety, dignity, and respect
Perform other duties as assigned by the nursing leadership team
Requirements
Current LPN Licensed Practical Nurse license in the state of Vermont
High school diploma or equivalent required; associate's or bachelor's degree in nursing or a related field preferred
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Strong attention to detail and ability to maintain accurate records
