LPN Licensed Practical Nurse Responsibilities:

• Provide direct patient care, including administering medications, wound care, and vital signs monitoring

• Develop and implement individualized care plans in collaboration with other healthcare professionals

• Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions

• Participate in quality improvement initiatives to continuously improve patient outcomes and satisfaction

• Collaborate with other departments, such as therapy and dietary, to provide comprehensive care

LPN Licensed Practical Nurse Requirements:

• LPN license in the state of New York

• 1 year of experience in long-term care or related field preferred

• Strong communication and interpersonal skills

• Ability to work effectively in a fast-paced environment

• Compassion, empathy, and a commitment to delivering high-quality patient care
