Position Summary:
We are seeking experienced and dedicated Registered Nurses (RNs) Licensed Practical Nurses (LPNs) to provide skilled nursing care to homebound patients on a 1099 contractor basis. This role is ideal for independent, organized clinicians who want flexible scheduling and the ability to deliver personalized care without the constraints of a hospital setting.
Key Responsibilities:
Perform comprehensive patient assessments and develop individualized care plans
Coordinate the development of a written plan of care for each resident that identifies the problems/needs/concerns for the resident. Check for adequate description of problems, resident centered goals, which are realistic and measurable and multi-disciplinary approaches to goals.
Chairs resident care plan conference and relays all concerns from resident and/or family/responsible party to the Administrator.
Oversee that residents/ families/responsible parties are contacted in relation to participation in care plan conferences per RAI guidelines.
Provide skilled nursing care including wound care, medication management, catheter care, and disease education
Document visits accurately and promptly in accordance with payer and agency guidelines
Monitor resident’s medical record, including MDS, Cardex, Care Plans, progress notes, Physician Order Sheets, Flow records, monthly nursing summaries, IDT notes/assessments, etc. for consistency and content. Address discrepancies in documentation which are not consistent and/or do not meet program standards to appropriate department manager and Administrator.
Maintain ICD-9 codes for current diagnosis.
Communicate changes in patient condition to physicians, caregivers, and care team
Supervise LPNs and HHAs as needed under CHC program guidelines
Collaborate with agency staff and case managers to coordinate care
Maintain professional standards and regulatory compliance
Travel to patient homes within assigned territory
Qualifications:
Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Pennsylvania
Experience with Therap preferred
Minimum of 2 year of clinical experience, preferably in home health, hospice, or long-term care
Reliable transportation, valid driver’s license, and auto insurance
CPR certification required
Experience with CHC programs or credentialing with PA Health & Wellness and Keystone First is a strong plus
Proficiency with electronic documentation (EMR) preferred, but not required
Strong interpersonal and time management skills
Ability to work independently and exercise sound clinical judgment
