About the Role:

As a Licensed Practical Nurse (LPN) on our evening shift, you'll be responsible for providing direct patient care, administering medications, and assisting with daily living activities. You'll work closely with our interdisciplinary team to ensure the highest level of care for our residents. Responsibilities:

• Provide direct patient care to include vital signs, wound care, and treatment administration
• Administer medications as prescribed by physicians
• Assist with daily living activities such as bathing, dressing, and toileting
• Develop and implement individualized care plans
• Collaborate with the healthcare team to ensure comprehensive care
• Maintain accurate and detailed records of patient care
