About the Role:

As an LPN, you will play a vital role in ensuring the highest quality of care for our residents. You will be responsible for providing direct care services, including administering medications, performing vital signs, and assisting with daily living activities. Your strong clinical skills, attention to detail, and patient-centered approach will make a significant difference in the lives of our residents.

Responsibilities:

• Provide direct patient care services, including administering medications, performing vital signs, and assisting with daily living activities
• Work closely with the nursing team to develop and implement individualized care plans for each resident
• Monitor and report any changes in resident condition to the nurse manager or physician
• Maintain accurate and detailed records of resident care and services
• Participate in care conferences and other team meetings to coordinate resident care
• Demonstrate a strong commitment to patient-centered care and a dedication to our organization's mission
