MDS Nurse

Hilltop Health in Rutherfordton NC is looking for a qualified MDS Nurse for our facility. RN license preferred. MDS experience required.

An MDS (Minimum Data Set) Nurse specializes in assessing, monitoring, and documenting resident health in long-term care facilities to meet state/federal CMS compliance, with emerging AI tools increasingly assisting in data gathering, auditing, and optimizing reimbursement. Key responsibilities include creating care plans, managing Medicare/Medicaid assessments, and coordinating with interdisciplinary teams.

Core Responsibilities & Job Description

• Assessment & Compliance: Conduct comprehensive, accurate, and timely assessments (admission, quarterly, annual) according to CMS regulations.

• Documentation & Coding: Utilize clinical data to code the MDS, directly impacting facility reimbursement and patient care plans.

• Care Planning: Develop and update individualized care plans by collaborating with physicians, therapists, and families.

• Audit & Quality Assurance: Review medical records to ensure accuracy, compliance, and identify potential quality improvement opportunities.

• AI Integration: Use AI tools to assist in finding Non-Therapy Ancillary (NTA) points, reducing claim denials, and summarizing patient data.

Key Qualifications

• Licensure: Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license.

• Experience: Usually requires 1-5+ years in long-term care or skilled nursing.

• Skills: Extensive knowledge of the Resident Assessment Instrument (RAI) process, strong attention to detail, and proficiency in electronic health records

• Teamwork focus: Requires the ability to work with staff at all levels of hierarchy from housekeeping to Administration and family members.

• Teaching: Has the ability and mindset to teach employees how their care and documentation of such effects the MDS and resulting CMI and Star ratings.
