Clinical Liaison – Long Term Care
Location: Scranton, PA Market (Scranton and surrounding areas)
Salary: DOE
We are seeking a dynamic Clinical Liaison to support referral development and admissions growth within the Scranton, PA market. This role focuses on hospital partnerships, community outreach, and referral management to drive census and strengthen relationships across the region.
The ideal candidate is a relationship-driven healthcare professional with a strong skilled nursing background and proven sales/marketing experience.
Key ResponsibilitiesProvide onsite coverage for referrals within Scranton-area hospitals, including Wayne Memorial and Dickson City
Build and maintain strong relationships with hospital case managers, social workers, physicians, and referral sources
Conduct clinical review of referrals to determine appropriateness for skilled nursing placement
Assist with referral management and admissions coordination
Support community marketing efforts to increase visibility and referral opportunities
Collaborate with facility teams to ensure smooth transitions from hospital to SNF
Identify opportunities to grow census and improve referral pipelines
SNF (Skilled Nursing Facility) experience required
Sales and marketing experience required
LPN preferred
Strong understanding of post-acute care admissions and referral processes
Ability to evaluate clinical needs and coordinate appropriate placements
Excellent communication and relationship-building skills
