CareDiem Home Health is seeking compassionate and reliable RNs to provide skilled home health nursing services to patients in ROCKFORD & SURROUNDING AREAS. Our mission is to help clients remain safely at home while receiving top-quality, personalized care.

What You’ll Do:

• Deliver skilled nursing care under a physician’s order

• Complete assessments, medication administration, and wound care

• Monitor and document patient progress using our electronic charting system (Axxess)

• Educate patients and families on health conditions and care

• Communicate with physicians and the care team

• Travel to patient homes

What We’re Looking For:

• Current RN license in the state of Illinois

• 1+ year nursing experience (home health preferred)

• CPR certification (BLS)

• Reliable transportation, valid driver’s license, and auto insurance

• Strong clinical judgment, communication, and independence
