About the Role

As an LPN or RN, you will provide direct resident care, including assessments, medication administration, and treatments. You will collaborate with the interdisciplinary team to develop and implement individualized care plans, monitor resident progress, and ensure optimal clinical outcomes. Strong communication and teamwork are essential to support residents, families, and healthcare providers.

Responsibilities

• Provide direct resident care, including assessments, medications, and treatments

• Develop, implement, and update individualized care plans in collaboration with the healthcare team

• Perform ongoing assessments and evaluations to ensure quality outcomes

• Communicate effectively with residents, families, and clinical staff to ensure continuity of care

• Maintain accurate, timely, and complete documentation in accordance with facility policies and regulations

• Participate in quality improvement and patient safety initiatives

Requirements

• Minimum 1 year of experience preferred (new graduates are welcome to apply)

• Current and active Illinois LPN or RN license in good standing

• Strong clinical, communication, and organizational skills

• Ability to work effectively in a team-oriented long-term care environment
