Position Summary:

LPNs are an important part of Delaware Oaks! Not only do they help our residents to achieve their goals, but they also help them to maintain an optimal quality of life. We're looking for caring, compassionate LPNs to join our team and to help us fulfill our mission to deliver the area's BEST senior care!

LPN provide direct nursing care to our residents, work alongside a collaborative team, and help ensure that our residents receive the highest quality of care in a safe and supportive environment.

Qualifications for Licensed Practical Nurses (LPN):

  • Licensed Practical Nurse (LPN) license

  • Compassionate, patient-focused, and committed to providing high-quality care.

  • Ability to work independently and collaboratively within a team.

  • Excellent communication and interpersonal skills.

  • Ability to maintain accurate records and handle sensitive information confidentially.

Responsibilities of Licensed Practical Nurses (LPN):

  • Administer medications and treatments as prescribed by physicians.

  • Monitor and document residents' health conditions, including vital signs, symptoms, and responses to treatments.

  • Assist in the development and implementation of individualized care plans.

  • Provide nursing care for residents, including wound care, catheter care, and assistance with daily living activities.

  • Collaborate with physicians, RNs, and other healthcare team members to ensure coordinated and comprehensive care.

  • Educate residents and their families about health management and procedures.

  • Ensure that all care is delivered in accordance with regulatory and facility guidelines.

  • Respond to emergencies and provide critical care when necessary.

  • Assist with training and supervising nursing assistants (CNA).

Key Requirements Other:

  • Participation in scheduling, being flexible, and willing to cover shifts as necessary.

  • Ability to provide direct resident care and provide clear instruction to care partners.

  • Ability to listen to resident, family, or physician concerns and implement appropriate action.
