About the LPN Licensed Practical Nurse Role:

• Provide direct patient care to our residents, including administering medications

• Develop and implement personalized care plans, working closely with our interdisciplinary team of healthcare professionals

• Maintain accurate and detailed records of patient care, medication administration, and treatment plans

• Collaborate with fellow healthcare professionals to ensure continuity of care and effective communication

• Participate in Quality Improvement Initiatives to enhance patient outcomes and satisfaction

LPN Licensed Practical Nurse Requirements:

• Current LPN licensure in the state of New York

• Prior experience preferred, new graduates welcome!

• Strong communication and interpersonal skills
