The Personal Health Nurse will play an important role in participant wellness, coordination of care, advocacy, education, and helping individuals remain independent and thriving in the community.

We are looking for someone who leads with both clinical excellence and heart.

Ideal candidates:
• RN licensed in Delaware
• Strong communication and relationship-building skills
• Passion for serving older adults
• Organized, proactive, and team-oriented
• Community health or care coordination experience preferred
