LPN -Part Time 7a-11am and 3p- 7pm

About the Role:

As an LPN at Meriden Health and Rehab LLC, you will be responsible for providing direct patient care under the supervision of licensed nurses. Your duties will include:

  • Administering medications, treatments, and therapies as prescribed by physicians

  • Assisting with daily living activities such as bathing, dressing, and grooming

  • Monitoring vital signs and reporting any changes to licensed nurses

  • Maintaining accurate and up-to-date records of patient care and treatment

  • Collaborating with the interdisciplinary care team to develop and implement individualized care plans

  • Providing emotional support and comfort to patients and their families

Responsibilities:

  • Provide direct patient care in a skilled nursing facility setting

  • Develop and implement individualized care plans in collaboration with the interdisciplinary care team

  • Administer medications, treatments, and therapies as prescribed by physicians

  • Monitor and report patient vital signs and changes in condition

  • Maintain accurate and up-to-date records of patient care and treatment

Requirements:

  • Current LPN licensure in the state of [State]
