Pay: $32.50–$36.00/hour (base rate; shift diffs & incentive pay available)
Schedule: Full-Time (3–11 Shift)

Job Summary:

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Apple Rehab Middletown, a trusted and family-owned rehabilitation and skilled nursing network dedicated to exceptional resident care. As an LPN Charge Nurse, you will play a vital role in coordinating high-quality nursing care for our residents while maintaining compliance with facility policies, safety standards, and state/federal regulations.

Key Responsibilities:

• Provide compassionate, professional nursing care to residents
• Administer medications and treatments safely and accurately
• Maintain clear, thorough documentation within regulatory guidelines
• Communicate effectively with families, physicians, and interdisciplinary staff
• Support a positive and collaborative team environment
• Ensure all care aligns with resident needs and medical best practices

Requirements:

• Valid Connecticut LPN license (in good standing)
• Knowledge of long-term care practices and state regulations
• Strong organizational and multitasking abilities
• Excellent communication and teamwork skills
• Long-term care experience preferred but not required

Additional Expectations:

• Weekend on-call participation required (compensation available)
• Per diem staff must work at least two weekend shifts per month
